GDPR consent for processing personal data in an app

Because personal data and health data are processed within the app, a GDPR legal basis of “consent” is required for this.
It is important that participants give explicit consent for the processing of their data, for example by means of a mandatory checkbox.
The most workable solution seems to us to be that participants see a notification or screen the first time they log in, requiring them to give mandatory agreement/permission before they can proceed in the app.
An example text could be:
I consent to the use of my data, including health data such as weight, diet, and exercise, by the designated coaches for guidance and advice.
