Trial lesson form with flexible fields + schedule instantly (incl. extra participants & “no times” notification)

As a coach, I want to be able to create a trial lesson form with flexibly configurable fields (per location/offer). After filling it out, someone should be able to immediately select available trial lesson times and optionally bring colleague(s) along (number + name/contact per participant).

If there are no suitable times, there must be a clear button with the exact text: “My time is not listed”.
Clicking on that opens a short request form (preferred days/times + comment) and creates a trial lesson request + immediately sends a notification to the coach (so you can schedule manually or release times).

Extra: when setting up this trial lesson form, I want to be able to choose which intake form is linked to it, and set per trial lesson flow whether this intake form is mandatory (must be completed before confirmation) or optional (complete later / after booking trigger).
